Attendant Care Coordinator

Job Description:
Responsible for assisting the Operations Manager in the management of the day-to-day operations of the Attendant Care Program. Supervise, support, and instruct Case Manager Supervisors in the provision of the Attendant Care Program.

Key Responsibilities:

  • Assigning work, following up with completion, review, and reporting case assignments
  • Recruiting and evaluating case management staff
  • Provide orientation and training to new case management staff
  • Provide guidance and assistance to case managers to refer consumers for proper services
  • Supervise and develop new staff to meet company objectives
  • Participate in advisory committees, as needed
  • Adhere to company policies and procedures

Requirements:
Candidate should have at least 3 to 5 years of experience supervising case managers for a home health care agency. In addition, candidate should have knowledge and experience on the preparation of individual service plans (ISPs) and HCSIS plans. Must also have experience working with governmental and social service organizations.

Masters degree in Social work or related field required. Minimum of five (5) years experience in social service/ health care field required. Must have three (3) years demonstrated supervisory and administrative skills. A car is required for this position.
