Case Manager Supervisor

Job Description:
Responsible for the management of delivery of in-home services in accordance with consumer’s needs including: referral to community resources and Director of Nursing. Reports to and is supervised by the Attendant Care Coordinator. Provides administrative support and assists the Attendant Care Coordinator.

Key Responsibilities:
Each Case Manager Supervisor must monitor the delivery of service via field supervision and consumer monitoring.

Each Case Manager Supervisor is required to adhere to the Attendant Care Program of the Department of Public Welfare requirements as it relates to service delivery and documentation. Evident in assessment, individual service plan, and service visits as reviewed by Attendant Care Coordinator’s and Operations Manager’s Consumer Record Review.

Each Case Manager Supervisor must adhere to the Department of Public Welfare’s requirements to maintain an accurate tickler system. Evident by a monthly review by the Attendant Care Coordinator.

Bachelor’s Degree in Social Work, Social Sciences or related field. At least two (2) years experience in a social service agency or similar setting. Some experience in a medical setting preferred. Must have demonstrated supervisory, technical and administrative skills. A car is required for this position.

Working Conditions:
Working in a variety of home environments that include the agency’s place of business and in the public and private places of business.

Supervised by the Attendant Care Coordinator.
