Nursing Home Transition (NHT)

Welcome Home!

Nursing Home Transition (NHT) is a program that offers clients a choice of where they want to live. NHT serves as a guide to help clients through the process of leaving a nursing home and returning to the community. NHT assists you in making an informed decision with whatever choice you make regarding where you want to live.

The NHT program is a collaborative effort of the Pennsylvania Departments of Welfare, Aging, the Governor’s Office of Health Care Reform; local County Assistance Offices, local Centers for Independent Living; home and community-based providers; and organizations representing nursing homes.

HSMA is certified by Medicare and Medicaid.

What We Offer
  • Resources to help our client build the lifestyle of choice
  • Provide assistance in finding housing
  • Planning the move and setting up the home, including adapting an existing home
  • Help with establishing a household and procuring the services necessary for independent living
How We Can Help
  • Advocate on your behalf.
  • Provide individual support and direction by trained case managers.
  • Assist in locating housing.
  • Assist with such items as security deposity, household items, adaptive equipment, utilities and other necessary items to help establish basic living arrangements.
  • Make referrals to home and community-based services.
Eligibility Requirements
Nursing home residents who wish to live in Bucks County, Chester County, Delaware County, Montgomery County or Philadelphia County.
