Support Coordination

While maintaining your independence and responsibilities, we can help to connect and coordinate services within the Attendant Care, Act 150, and Independence Waiver Programs. In addition, we can assist to connect and coordinate services outside of the programs to further assist you with your needs.

Our goal is to allow each consumer to live comfortably in their own home by providing high quality support coordination which allows consumers to meet their needs. Case Managers provide support coordination by completing the following:

Assessment
An accurate evaluation of a consumers’ needs, preferences, support systems, and desired outcomes.

Individual Service Plan Development
With the participation of consumers, create an individual service plan that meets the consumer’s needs and goals.

Referral
Assist the consumer with selecting qualified providers and make arrangements to ensure that providers follow the individual service plan. In addition, assist the consumer with connecting to community resources as needed.

Monitoring
Provide ongoing case monitoring to ensure that the consumer receives authorized services and that the services continue to meet the needs of the consumer.

Remediation/Resolution
Assist the consumer in resolving problems as well as anticipate the potential for problems based on identified needs and risk factors.

Eligibility Requirements

Attendant Care Waiver Program Eligibility Criteria

  • Must be enrolled and approved for ACW program
  • Must be between the ages of 18-59
  • Must have a physical disability that is expected to last one year or longer
  • Must be mentally alert and able to direct own care
  • Must be Nursing Home eligible

Services include: Support Coordination for Personal Assistance Services, Financial Management Services, and Personal Emergency Response System


 


 

Act 150 Program Eligibility Criteria

  • Must be enrolled and approved for the Act 150 program
  • Must be between the ages of 18-59
  • Must have a physical disability that is expected to last one year or longer
  • Must be mentally alert and able to direct own care
  • A fee determination will be completed based on household income

Services include: Support Coordination for Personal Assistance Services, Financial Management Services, and Personal Emergency Response System


 


 

Independence Waiver Program Eligibility Criteria

  • Must be enrolled and approved for the Independence Waiver program
  • Must be between the ages of 18-59
  • Nursing Home Eligible
  • Must have a physical disability that is expected to continue indefinitely, Physical disability that results in at least 3 substantial functional limitations (Mobility, Self Care, Communication, Learning, Self Direction, Capacity for Independence Living)

Services include: Support Coordination for Personal Assistance Services, Financial Management Services, and Personal Emergency Response System, Community Transition, Non-Medical Transportation, Assistive Technology/Specialized medical equipment and supplies, Community Integration including Educational Services, Environmental Modifications, and Skilled Nursing (Wound Care, Equipment Training, Therapies – Physical Therapy, Occupational Therapy, Speech Therapy and Behavioral Therapy)
