PRN Nurse Case Manager (Day Shift)

Job Description
As a part of a multidisciplinary team, the nurse case manager will provide skilled nursing services to home bound patients in Philadelphia. The nurse case manager will evaluate the patient’s physical and environmental health status to implement an effective care plan that meets the need of the patient, caregiver and family. The nurse case manager will coordinate patient care with members of the multidisciplinary team. The nurse case manager will accurately document within a required time frame in accordance to agency guidelines.

Key Responsibilities:

  • Assures quality of care by adhering to therapeutic standards; measures health outcomes against patient care goals and standards; making or recommending necessary adjustments; follows philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations.
  • Maintains continuity among nursing team by documenting and communicating actions, irregularities, and continuing needs.


  • Experienced Registered Nurse with a minimal of 2 years Home Health Nursing experience
  • A valid, current RN license in the Commonwealth of Pennsylvania
  • Current CPR certification
  • Strong assessment and organizational skills
